Your prescription must be less than 12-months-old. You can Fax it to us or email a copy to info@sightmakers.com.au

Regarding Your Optical Prescription
Your prescription information must also include the Pupil Distance (PD) measurement. Your optometrist has your PD measurement on file, and will supply it to the measurement to you upon request. Sightmakers cannot be responsible for incorrect or incomplete prescription information. Therefore we will require a photocopy or fax copy of the actual prescription.
